Weight Change Plan
Changing your body weight in a healthy direction IS achievable. The most effective, lasting, and safe ways of getting there are the Wellivery ways!
This 15 minute virtual encounter allows me to assess your current weight and chronic disease risk status. Together, we will develop and launch a plan for change!
- HOW WE CONNECT:
- I like to do the first visit using a video format such as FaceTime, Skype, or Zoom.
- Subsequent online doctor visits with Established Patients can usually be completed by phone.
- For the first appointment, I will send a video link to your phone at the day and time of your appointment.
- HAVE THESE THINGS READY:
- A picture ID.
- A copy of your most recent medical documents, lab reports, specialist consults
- Your blood pressure for the past few days.
- Your Height.
- Your Body Weight.
- GOALS FOR THIS SESSION:
- Establish a preliminary plan
- Order any labs or other medical history documents that are needed.
- Medication Cost:
- If you are having difficulty paying for your medication, please let me know as many of the pharmaceutical companies offer special pricing and in some cases free medication.
