This 15 minute virtual encounter allows me to assess your opiate dependency and chronic disease risk status. Together, we will develop and launch a plan that will help you quit!
- HOW WE CONNECT:
- I like to do the first visit using a video format such as FaceTime, Skype, or Zoom.
- Subsequent online doctor visits with Established Patients can usually be completed by phone.
- For the first appointment, I will send a video link to your phone at the day and time of your appointment.
- HAVE THESE THINGS READY:
- A picture ID.
- A copy of your most recent medical documents, lab reports, specialist consults
- Your blood pressure for the past few days.
- Your Height.
- Your Body Weight.
- GOALS FOR THIS SESSION:
- Collect your health history
- Establish a preliminary plan
- Order any labs or other medical history documents that are needed.
- Order your first round of prescription medication
