Colorado Medical Marijuana Certification

If you have a qualifying medical condition under Colorado law, I will meet you in my office to conduct a physical exam and review your medical history. Once I have confirmed the existence of a qualifying condition, we will review the risks and benefits of medical marijuana for your condition. If you choose to proceed, I’ll issue a physician certification.
The State of Colorado has an organized system for medical marijuana certification. At its core, the State maintains an electronic registry where I enter your:
- qualifying medical condition
- dosage
- the amount and forms of marijuana authorized
An IN-PERSON encounter is required. At this encounter, we'll review your medical history and I'll complete a physical exam. I'll review medical history documents you have obtained. Based on this encounter, we'll develop:
- A problem list (your current medical problems), and
- A treatment plan to address your problems.
- We'll review the risks and benefits of medical marijuana for treating your qualifying medical conditions.
- If you elect medical marijuana as a treatment option:
- I'll have you complete and sign the standard Informed Consent form,
- I'll enter you into the State registry,
- I'll schedule you for a 3-6 month follow-up visit to assure your treatment plan is working.
My goal for you is to NOT ONLY provide a review of your health status to certify you for MMJ. Meeting you, listening to your history, examining you, and developing a health plan for you means I am invested in your whole-health. I am prepared to be your primary care doctor and will deliver both routine health care needs as well as chronic disease screening and treatment coordination. I provide this care at an Affordable price, recognizing the difficulties patients face in paying for health insurance and routine medical care.

How will your Appointment work?
After you make your online appointment, I'll contact you to have you complete a medical history form and acquire medical history documents that pertain to your medical condition.
BRING THESE THINGS TO YOUR APPOINTMENT:

- A picture ID.
- If you are taking a prescription medication, please bring the bottle(s) with you.
- Please have your preferred pharmacy in mind so I can electronically transmit future prescriptions.
- NEW PATIENTS:
- My goal for you is to arrive at a preliminary working diagnosis
- To do so, I will need medical records from doctors you have seen previously.
- I’ll send you a Release of Information form to complete and send to those doctors.
- They will fax me your records and I’ll add them to your electronic chart.
- Many times, I can determine your working diagnosis by hearing about your past problems
- Your physical exam will be used to detect other medical problems and to confirm your established problems.
- Together, we'll develop a Medical Problem List and a Treatment Plan that is personalized for you. This Plan will address:
- your current medications
- screening for other diseases
- the role medical marijuana plays in your health.
- If you elect Medical Marijuana as a treatment option, we'll complete the process described above.
- Medicine prescriptions that your require will electronically transmitted to your preferred pharmacy
- I will direct you to educational materials.
- We will arrive at a follow-up day for your next appointment, usually in 3-6 months.
- At that appointment, we will discuss your response to your Treatment Plan and adjust it as needed.
- My goal for you is to arrive at a preliminary working diagnosis
